If you would like us to fill out school/camp/other forms, please note:

1. There is a $25 fee payable at drop off unless form is filled out during an office visit, cash or check only.

2. Please fill out the patient’s name, date of birth, and list which medications are needed on the form and list patient’s allergies. We will fill out the rest. Any information not filled out may cause the form to be returned or a delay in processing.

3. Due to high volume, forms may take up to a week to be filled out by the doctor.

4. We will call you when the form is ready for pick up. If you haven’t heard from us in a week, please call to check on the status.
