A true drug allergy involves an immune-mediated response to a medication. However, many reported drug allergies—including the majority of penicillin allergies—are not true IgE-mediated reactions. Adverse drug reactions, medication side effects, intolerances, and remote childhood reactions that were never properly evaluated are often mislabeled as allergies. Accurate diagnosis is essential for receiving the most effective medical care.

How We Diagnose

Testing may include penicillin skin testing, which remains the gold standard for evaluating penicillin allergy, skin testing and graded challenges for selected medications, drug provocation challenges to confirm tolerance when appropriate, and assessment of cross-reactivity among related medication classes such as cephalosporins and carbapenems.
